Specialist Support Coordinator

(Kadina, Clare or Nuriootpa)

We are seeking an experienced Support Coordinator to join our growing Support Coordination Team.  This position can be based in Nuriootpa, Clare or Kadina, and will work across the regions of CADL.

This exciting opportunity is being offered as a permanent part- time position of up to 22.5 hours per week with the view to grow the service resulting in an increase of work hours.

Applicants possessing elements listed below coupled with the intrinsic requirements of the Position Description will be highly regarded.

  • An ability to work collaboratively with other disability service providers under NDIS plans
  • An understanding of coordination of supports for NDIS participants
  • Experience in psycho-social disability, disability housing or SIL
  • Leadership, team work, planning and relationship building abilities
  • A demonstrated understanding of the potential needs, barriers and opportunities of people living with disability and their carer/s
  • A demonstrated ability to meet personal goals, client driven outcomes and organisational  targets

The terms and conditions of this role are set against the SCHC&DS Industry Award, and the successful applicant will be employed at Level 4.

As demand for disability services has increased across our communities and we are looking for highly motivated, passionate and self-driven individuals to support our clients in their homes and communities.

Minimum current qualifications required are:

A current Department of Human Services (DHS) Disability Services Employment Screening clearance AND if you wish to work with children, you will need a Working with Children Check.

First Aid Certificate; Manual Handling Certificate; Food Safety Certificate;  Infection Control Certificate, ABN.

Other qualifications and skills in disability, aged or home and community care, education, or other relevant discipline, will be an advantage.

Subject to your skills and qualifications, Independent Contractors may be required to deliver any of the following services:

  • personal care
  • meal preparation
  • domestic duties
  • gardening
  • respite/social support
  • community support
  • assistance with development of independent skills
  • mentor
